Enrollment Process

In order for students to participate in the Indian Education program under the Federal Government Title VII Eligibility, the student's parent/guardian must complete the Title VII Student Eligibility Form: 506- Form. Completing this form enables your student to be counted for funding under this program.

This form will be maintained at your child's school and will no be released without your written approval. This form will become part of your student/child's school record and will not need to be completed each year. The form can be found at the link below.
